Application Forms

Does anybody have an application form they use for hiring employees that they are willing to share?
Something that is good and something that protects the interest of the company?
Or does anybody have a link that has some good free ones? Thank you.:grin:

Be careful about using the previous employer form.
As a previous employer, you are only allowed to answer questions that do not require an evaluation.
Basically, you can give name, rank, and serial number.
Anything beyond that can open you up to a lawsuit.

Not to be a spoil sport or anything, but there are slight differences from state to state in what is considered acceptable on a job application.

A lot of states unemployment departments have generic forms you can download off their web site.
