Make your own on Microsoft Spreadsheet. This way you can customize the line items to fit your needs. A purchased or "borrowed" form are somewhat be too general for most contractors.

Would be pleased to. My associates were so pleased with the initial version that I'm polishing it up a bit. Should be done this weekend.
