What year/code cycle did 4 wires for stoves start?

The 1996 NEC section 250-60 was when the change was made .Mobile homes ranges and dryers were required to be four wire prior to the 1996 NEC.

for manufactured homes, Hud CFR 24 3280.800 which referenced the 1993 NEC required it in 1994:

CFR 24 3280.809 (2) Connection of ranges and clothes dryers with 120/240 volt, 3-
wire ratings shall be made with 4 conductor cord and 3 pole, 4-wire
grounding type plugs,
or by type AC metal clad conductors enclosed in
flexible metal conduit. For 120 volt rated devices a 3-conductor cord
and a 2-pole, 3-wire grounding type plug shall be permitted.

Maybe the real question is when did it become an issue that you could not use a 3 wire connection from a sub panel.

The answer is I think never
