Breaker Panel door

I have a installation that has a 120v 24 circuit breaker panel that has the main cover intact but has had the door removed. It appears this was done becuase there is a pipe that protrudes in such a way that the door would not open freely (lacks about 2 inches). This is not a new installation, the panel has been in place for about 25 years and appears this was done during construction. Does the hinged door need to be replaced or is there a violation present?

The hinged door is optional. The dead front is required.
The are many panels sold that do not have doors.

110.26 Spaces About Electrical Equipment.
(2) Width of Working Space. The width of the working space in front of the electric equipment shall be the width of the equipment or 750 mm (30 in.), whichever is greater. In all cases, the work space shall permit at least a 90 degree opening of equipment doors or hinged panels.
