There is no such requirement. The requirement, as provided in 110.26, is that the working space remain clear. If two items are stacked one above the other, and one sticks out more than 6 inches beyond the other, that could be an interference with working space. But if, for example, you place a transformer above a panel, and the bottom of the transformer is more than 6.5 feet above the floor, it can stick out well beyond the face of the panel, because it would not be within the panel's working space.
