Does anybody out there know If there is a stipulation anywhere in the code that prevents using an EMT nipple to protect what would otherwise be exposed MC cable from physical damage? ie: running from an under floor location to a surface mounted j box on a CMU wall.

As Dennis stated MC cable is permitted in raceways as long as you maintain the proper conduit fill. A sleeve used for protection is not a raceway so fill requirements will not apply.

A sleeve with the cable leaving both ends exposed is understandable.

A sleeve of raceway that terminates into a box, cabinet, conduit body, etc. -- does it need to have some kind of raceway to cable adapter, or can the cable just be run into the raceway? If so where can the end of the sheath be?

I have always used a coulpling and MC connector to make an adapter when doing this and stripped all of sheath off of conductors beyond that point.
