What is the smallest size of grounded conductor that does not have to be white or gray for its entire length?
N.E.C. 2008 Edition states in Section 200.6 B-3: "Sizes larger than 6 AWG (at the time of installation, ...a distinctive white or gray marking at its termination. this marking shall encircle the conductor or insulation.".

Here's the entire section relating to larger than #6 AWG conductors:

200.6(B) Sizes Larger Than 6 AWG. An insulated grounded conductor larger than 6 AWG shall be identified by one of the following means:
(1) By a continuous white or gray outer finish.
(2) By three continuous white stripes along its entire length on other than green insulation.
(3) At the time of installation, by a distinctive white or gray marking at its terminations. This marking shall encircle the conductor or insulation.

For some reason that section has been misconstrued by many people to include #6. So, 2011 has a small change.

(B) Sizes 4 AWG or Larger. An insulated grounded conductor
4 AWG or larger shall be identified by one of the
following means:
