How many conduits can penetrate a 4x4 box before you have to add an extension ring.

I have been trained once you penetrate a 4x4 more than 4 times you have to add an extension ring. Now I have been told that it in not required. What is propper etiquette.

How many Kos does it have?

It is conductor fill not raceways that fill up a box.

It is possible to only have one or two entries and need an extension.

I suppose you were tought a general rule of thumb, The code does not work that way. You go by wire fill and each conductor counts as per the tables , the grounds usually count as 1

4 X 4 are only two of the dimensions you need. How deep is it? Then of course all the other reasons already stated.
