My interpretation of the code you cited is that if your water heater is installed above the ceiling, then your disconnect is allowed to be greater than 6'7" from the floor as long as you install the switch adjacent to the water heater. Adjacent means "next to" or "in proximity".

IMO, being separated by a suspended ceiling does not meet the definition of adjacent.

As I see it, the exception you quoted would allow you to place the switch as jeremysterling notes, but you would not be required to place it there. 422.31 would require the disconnect means to be within sight or lockable, and 110.22 would require it to be identified, unless obvious,
