Can anyone here recommend a good old work style fan box. I have got several fans to install in vaulted ceilings(5/8 drywall) that I don't have access from above. I can fish the cable to the box with glow sticks but can't physically get up there to mount from attic space. Years ago I recall using a product that was very poorly designed, I don't remember the brand but would know it if I saw it. Thanks.

I have a couple of those installed in my own home. They work well and in the past I have even been able to shorten the cross bar for joists that were closer together than normal.

I have done that also. Not sure what brand it was but I did have to cut about 4 inches since the joist were 12" on center instead of 16. It worked

Dennis nailed it, best product I know of. There are others with a screw instead of the claw design, but the screw bends as it goes into old hard wood.

Jumper and the guys have it right-on.

And, as Jumper says, the spikes work best.
I have put my whole body weight on one just for a test. It held.
The stand-offs can be trimmed, and the bar shortened as needed.
