In kitchens, receptacles other than countertop receptacles do not need to be GFI receptacles?

For the 6 foot rule of receptacles located near sinks, is this only for sinks other than kitchens so even if a receptacle is 2 feet away from a kitchen sink but it is underneath the counter top, would it required to be GFI or does this 6 foot rule only apply to utility sinks and bar sinks?

Is this correct for the following receptacles required to be GFI:

All kitchen countertop receptacle: Y
Kitchen receptacle 18” receptacle off the floor: N
Kitchen receptacle underneath cabinets: N
Kitchen receptacle above cabinets: N
Kitchen receptacle underneath cabinets and 2 feet from the sink : N
Kitchen receptacle above cabinets and 2 feet from the sink : N

Under the 2014 NEC all receptacles serving the counter top surfaces must be GFCI protected. Also all receptacle within 6' of the sink must be GFCI protected. And the dishwasher must be GFCI protected.

What about the receptacle for the Garbage Disposal or for an above oven Microwave where the receptacle is in the cabinet above the microwave?

Yes, within 6' regardless of location. In prior code editions it was possible to have the refrigerator or other wall receptacles within 6' of the sink but they still did not require GFCI protection. The 2014 has expanded the GFCI requirement.

Dishwashers are now required to be arc fault protected as well. They haven't even come out with that combination device yet. It's been a problem with remodels not so much with new construction. My loop hole has been PA still hasn't adopted the new code there still using 2008 but we comply with our inspectors who are current

Leviton has a dual function AFCI/GFCI receptacle.
