Installing a 400 amp service. From the power pole at the street the power company will come 60 feet to the first pole on your property. If you go underground from this pole 150 ft to the house do you have to have a 400amp disconnect at the first pole where you went underground or can you just run a conduit from the service attachment at the top of the pole all the way underground to the house?

Most likely, The point of attachment for the underground service will be where the wire is terminated on the line side of the utility meter. Therefore, no disconnect will be required at the pole.

Most utilities will not meter after a disconnect, and the service disconnect and the service OCD must be loacted with each other. (See 250.91)
