disconnect and protection after the transformer

hi! just a question, are the following options acceptable for a disconnect/protection of the transformer secondary rated 2MVA 13.8/0.230kV:
1. 5000A load break switch only
2. 5000A ACB with LISG
3. no main breaker, bus only
4. two 2500A main breakers

A couple of things to consider:
1. Sizing of the OCPD for transformers primary and/or secondary is based upon NEC 450.3.
2. What is the available fault current (short circuit current) at this point where the protective device is being placed?
