Is there any code or requirement for voltage, ACI rating, physical size of gear, or whatever that would call for at least a fenced in, lockable, dedicated electrical gear area where unauthorized personnel could not get access? If so, at what point should the line be drawn between needing and not needing this area?

AFAIK NEC mostly only requires some kind of barrier to keep unauthorized persons away from equipment with exposed live components. This is also something not really seen much if at all in 600 volts or less equipment.

Now other codes or safety standards (like 70E) may require more than NEC does in some cases.
