Just randomly thinking, would you have to make existing circuit say knob and tube or bx with no ground strap gfci protected if you added a few new lights in a room wired with new romex, or is there an exception etc for this type of scenerio.

DD, the NEC would require that any new work be done according to the code being used by your local or state building code requirements.

Any time you use a GFCI to add grounding receptacles to a non-EGC-equipped circuit, you should not connect any EGC to any receptacle.

410.18 is worth a look.
