They let the smoke out. It used to be said that it took smoke to generate the electricity and it was very important to keep the smoke from coming out the other end.
It is very important to keep the smoke in the wires and not let it out.

I'm guessing water got in thru the broken/missing cover, corroded the internals, and created a short between hot and neutral, resulting in what is pictured; receptacle burning from the inside out. I've seen numerous 250V 20A simplex receptacles do the same in PTAC sub-bases when the sleeve is damaged, allowing condensate to run all over the receptacle.

What is the imprint on the receptacle face in the upper left part of the pic? Doesnt appear to be an older WR designation.

Just never seen one cooked this bad.
I've seen many, especially where some rain water seeps in. Is it just me and my old computer that can't see the images in the OP ? I had to copy the image address and paste it into a new browser window.

if you look at the device, you can clearly see there was a warning
that was not heeded.

well, you can clearly see the warning, just not what it was. that
part has degraded over time.
