Transformer secondary

Does article 240.21 (C) (4) intend that 2 separate main breakers can be fed from a common transformer secondary so long as the conductors for each breaker have an ampacity equal to or greater than each main breaker rating/setting and the conductors originate outdoors?

As long as the transformer has the required protection noted in Art 450.3
you can make your statement and exclude the word "outdoors" if you obey the corresponding tap rules.
In other words, a properly protected (primary) transformer can feed multiple OCP devices on the secondary as long as each is installed per 240.21(C) ___ tap rules.
(C)(4) simply removes any length restrictions {such as the 10 ft in (c)(2)}
provided the provisions of (C)(4) are met.
