I have a one-line drawing that has by each transformer a ground symbol with a note stating conduit and wire size and the wording "To code Grn". I assume this means a separate conduit and wire back to the main Service switchgear grounding buss?

I assume you mean dry type transformers through out the project and it would mean to the closest code grounding electrode such as building steel or water piping, or other approved electrode per exception #1 for an SDS, see 250.30(A)(4)

If the main service switchgear GEC is installed properly to the building steel, then the building steel can be utilized anywhere in the building for a GEC such as your transformers via simple mechanical lugs.
I typically install the required conduit size and conductor from the transformer straight up the wall to the building steel at each XFMR location.
