We’ve seen enough faulted conductors in earth including phase to phase to know how they fail. Generally you get a hard concrete like clump, occasionally glass if it’s in sand. Never a neat small hole in one phase with missing conductor streaked above the hole and some puddled below. The trench was 7’ long with soot continuing on into the sand each way.could have just been a short circuit where insulation failed. they can be pretty spectacular.

This is likely a one time surge event (big surge) and that was a weak point in the insulation, maybe got nicked when being handled at/before installation even.
