Grounding Table Question

Hello, I am having a hard time understanding any scenarios where you would use the 250.102 table to size the ground conductors running along the phase conductors instead of the typical 250.122 table. I believe i have a read that the secondary side of the transformer before the disconnecting means since its not in the same enclosure as the SDS would require 250.102 table to be used to side the ground conductor (acts as the supply side bonding jumper). What about a generator as a SDS? Would this be 250.122 because disconnecting means is in the enclosure? Any other scenarios?

Also, is a GEC and EGC required on all SDS over 1000kVA?
