mounting strap to raised cover

I was going thru some old stock, and came across some 4 sq raised box duplex covers. These are the ones with only one mounting hole for the center of the duplex.

The newer style has three holes for the duplex which make for a more secure mount.

I realize the eq ground can not rely on the box for a raised cover, but was curious about your thoughts on these two styles, and their differences.

Are these older style covers obsolete?
I have often seen the newer three hole mount, raised (industrial) covers mounted with only one screw in the middle and thought it was wrong. Does the NEC speak of this other than "follow the manufactures listing instructions"?

406.4(C) is the relevant article. Industrial covers for duplex receptacles with only one mounting hole are obsolete. I would find a use for them in your own garage or basement.
