If an existing 208/120V electrical system served from a 480V to 208/120V transformer has an equipment ground conductor with all feeder and branch circuit conductors but does not have an isolated ground conductor, and needs one, can an isolated ground conductor be added in a separate conduit outside of the system circuit conductors? If so or if not, by what part the NEC.

It cannot be run in a raceway or cable that does also contain the circuit conductors. 250.146(D) and 300.3(B)

You can add one there, but it can't be the only EGC for that circuit,as the EGC has to be run with the circuit conductors.

If you want to add some IG receptacles, or change existing receptacles to IG receptacles, the IG Equipment Grounding Conductor will need to comply with 250.146(D). This section will require the conductor to be run with the circuit conductors.
