Floor box for 120V and 240V

I'm looking for a floor box that will accommodate a 5-20R and a 6-20R. 2 for new slab and 2 for existing 8" slab. (poke through?)

You just need a 2 gang floor box. Are you looking for a cover that has a duplex on one half and a simplex on the second? If you cant find that, you could use a duplex 6-20R, like a Leviton 8400.

Will you have access below or are you cutting the slab for a horizontal conduit?

There is access below, so a recessed poke through would be great.
