Years ago, while studing for my state license, our instructor asked us a question: The NEC has five (5) locations that require the use of blue colored insulation or conductors. What are they?

I don't think I ever found all 5, but I know at least one - 424.35-marking on heating cable.

Anyone got any ideas?
