fixture mounting

I have a question about mounting a fixture to a concrete ceiling. For years when I mount a fixture to an octagon box in a slab I put a cover with chase nipple and connect it to the fixture. I see alot of fixtures mounted to a ceiling with just a chase nipple on the fixture and not mounted to the box. Is this up to code with a ground from box to fixture and no other connection from fixture to box?

Let's start with the type of fixture. A flourecent fixture cannot be mounted over a box unless the conductors within the box are accessible after installtion of the fixture. Is this the type of fixture? 410.24(B)

Yes , I guess that's why they make these fixtures with a big hole in the middle.

Exactly. For years a 1/2" chase nipple in the center KO was SOP when installing these types of fixtures over concealed boxes. That method is no longer permitted.
