Working in manholes w/ live high voltage cables

Are there any requirements by OSHA or otherwise that identify a safe means for working in manholes with live open high voltage cables?

Open? Define open.

As far as your requirements as mentioned by Walton OSHA 1910.269 has tones of requirements to be met. As far as practical experience, we used to use arc flash supression blankets to set up a work area, there is not much in the 70E or OSHA on this (Although it is in the 70E task tables), the blankets just started getting rated under the new ASTM standard and should be addressed in the 2012 70E.

I've recently been exposed to two oppurtunites to work in medium voltage vaults. The technique used for safety that I have seen is pray to whatever god you believe in and don't touch anything... I honestly don't want to go in these vaults without a qualified lineman.
