Duct Bank Requirements

Is there a specific place in the NEC that indicates under what circumstances a duct bank is required?

It a design issue, after it becomes part of a design the installation might become a code issue.

There is also the issue of the POCO. There are times they will require a duct bank installed.
I am aware that different POCOs require different installation procedures.

Not so much a code issue, but a system integrity issue.In the Annexes (back of the code book) there are recommendations, but not code. For 277/480 I would think about a red mud cap, in addition to a scare tape above the first lift (backfilled and compacted 12"), might want to put in scare tape a foot below grade over pipes that are not capped with concrete.

Just a thought, as a person never knows what the ground maybe torn up for whatever reason...
