I have an 800 amp 3ph sevice. (4) 200's (main lug) throughout restaurant. All grounded conductors and grounding conductors are bonded in every panel. Most circuits are bonded under the same screw. I'm trying to decide if this service was ever legal? or did it slip by somebody.

In 1999. I would have thought that the requirement to separate N and G, except at the service, was much older than that. But I looked through older versions, trying to find a similar requirement in other sub-articles of 250, but could not find it.

Me too, I thought the requirement was way older than that. I looked through '99, '02, '05 '08 but '99 is the oldest one I have. That's surprising to me. So that tells me this was a legal install at an '85 inspection.
