sizing conduit for use & use-2 conductors and Ground loops

I have two questions: The first is what insulation do use to size a conduit for USE-2 cable when sleeving it in conduit. In the past I have always went with the largest insulation on the table because I did not know for sure. I have looked up construction and have not found anything. I would guess XHHW. Anyhow, saving a dollar is getting more and more important and I was trying to cure a question that has been ratteling in my mind. Question 2 is another that was explaned when I was in school but I am not sure if I understood it well enough. The Question is how does a ground loop bypass a breaker and what other problems are created from ground loops. I have tried to explan this my self and can't seem to make enough sense of it to explan it well.:-?

The conduit sizing is what I am struggling with (which is based on insulation type). Unless I am missing something?

Is the USE-2 part of a urd cable assembly?

If this is a multiconductor cable assembly then take a look at Note (9) to Table 1 of Chapter 9.
