Disconnect at the meter

I think it is always a good idea, but I do not think it should ever become a Code requirement for over 99% of the installations.

Exception (the 1%) would be large services with multiple meters (more than six) which IIRC is already covered by Code anyway.
 
Where I?m at the fire department pulls the meter.

I have ran across a few houses that had them and the home owner upgraded there 100 amp panel with a 200 amp panel using split bolt?s and cheap electrical tape to extend there under sized feeder wires.

That's why we have electrical inspectors and firefighters........
