warning labels

I have a key locked hoffman box with 575VAC 60Hz three phase, 120/208VAC 60Hz three phase, and 115VAC 400Hz three phase coming into it and distributed out of it. What do I need for voltage warning labels on this box and do they have to be on the outside of the locked cover or is it sufficient to have them on the inside of the cover seen only when unlocked and open?

Does it have a second cover on the inside, I would put it their. Put a danger arc flash and electrocution hazzard sticker on it. Lable the voltage.
Sorry I don't have the NEC reference.

I usually recommend listing only the 'worst case' conditions, this way anyone entering the box will have the correct PPE before entering the enclosure. Also, I would indicate if one voltage may still be present even though other ones have be de-energized.

A label inside the box can provide more information on each individual circuit to assist in troubleshooting.
