for table 230.55 I have a question for note 4.Can you use note 4 for a range,and wall mounted combonation.or does it have to be a cooktop,and wall mounted oven combo? then I just add nameplate of both,and for every kw over 12 that is an additonal 5% to 8000.

That's 220.55 - It's my understanding that it was developed for a stove top and wall oven.

Welcome to the forum.

You can't use a range and wall mounted oven together in accordance with Note 4.

You can add a counter mounted cooking unit and up to 2 wall mounted ovens.
