Isolated grounds would be a part of the design, not an NEC requirement.

Separate grounds does not always mean the same thing as isolated grounds.

Separate grounding just means that the dedicated circuit's grounding wire can't be connected to another circuit's grounding wire. That means that the dedicated circuit can't share an enclosure with a different circuit as the grounds would have to be connected together.

Isolated grounding means that the third hole is connected by an insulated wire that runs all the way back to the panel and is not conected to anything else, even metal likely to become energized. The yoke still has to be grounded, but can be done via conventional means and must be connected to metal that is likely to become energized.

Short answer is no. For whatever reason someone does not want to rely on the conduit for equipment grounding purposes so a separate EGC is pulled in with the circuit conductors. Properly designed IG systems are more involved than just pulling a separate EGC in the raceway and using an IG device.

A separate ground could be:
Redundant - an egc parallel with other recognized grounding methods (IE emt conduit)
Isolated - an egc isolated from other recognized grounding methods.
In the first case an isolated ground receptacle is not required. In the latter it is.
In either case the isolated ground is more of a design issue, however the redundant ground is required by code in one or two locations.
