Ltg-inst form what are they asking for ?

hello could someone explain on form LTG-INST what this statement is asking for it reads--in the table below identify all applicable construction documents that specify the requirements for the scope of responibility for this installation cretificate. the table below has 3 columns first says document title or description second column says applicable sheets or pages,tables,schedules,etc third column says date approved by the enforcement agency. any in put would be greatly appreciated

thanks, but i have the form, my question is how do i fill it out what type of information do i put on the form following the formentioned question thanks
