Metal handle vs Plastic Handle Disconnects

Are Metal handle disconnects a code violation? Have you noticed that the handles are more plastic?

AFAIK, metal handles are not a violation: but I thought all new ones required a insulated handle. I cannot find it in the UL white book, so maybe I am wrong.

Your first picture is from a disconnect that was probably built prior to 1980, a period when plastic was not as widely used as it is today.
Your second picture is from the mid 80's to present, a time when fabricating injection molded parts is more common than 'bent' metal.

IIRC, some of the cabinet disconnect handles we use have metal handles for everything but 4X applications where it changes to plastic.
