Bonding bushings

Where are bonding bushings needed coming to or from a transformer
Transformers are not part of the criteria for needing to use BBs.

The quick answer is service equipment and anything over 250V to ground, but the bonding requirement isn't limited to using only BBs.

See 250.97 for more info.

You might use one on the GEC if it enters the transformer enclosure but for the most part they're not required.

I seem to recall that the code used to require treating the secondary conductors/raceway of a transformer up to the first OCPD as service conductors for bonding purposes. Am I dreaming or possibly a senior moment?

Actually we do treat transformer similarly (GEC size, bonding jumper size) but not with regard to bonding the raceway containing the secondary conductors. For some reason this concept of bonding bushings on transformers comes up very often on the job site.

Same here. GEC size, bonding jumper, etc are similar to a service... but the raceway bonding before the first OCPD, I thought sure that it used to be there.
