NEMA 3-15 and 4-15 receptacles ?

This came up in email conversation recently:

We often see NEMA 1-15 and 2-15 receptacles (125v & 250v 15a 2 wire) in older buildings. Has anyone ever seen a 3- or 4-? I'm told that 3- is 277v 2-wire and 4- is 600v 2-wire (from an older IATSE exam), but I'm sure I've never run into those. Has anybody (in an old factory, maybe)? Were there even made?

Yes, 3- and 4- aren't in any of the current lists, but did they ever exist? If not, why were those numbers skipped? (Is there any rhyme or reason to the voltage number, anyway?)

Sorry, yes. Nema number 3 was for 277V, nema 4 was for 600 volt. both are 2 pole 2 wire
