Does an electric Hot water switch box need to be painted in red? Inspector is asking for red box, as far as I know only apply to gas or oil appliances.

And if required to be red for the oil or gas appliance it is not an NEC requirement but may be by other codes. I think you usually see this for oil burning appliances, if there is an emergency situation they want the oil pump disconnect easily identifiable so the pump can be shut off to prevent continuing to pump fuel into the situation.

the only red switch cover I've ever seen was on E/stop or shut off switchs. Fire alarm is generaly red.

I have never heard of a red disco for a water heater. Does he req. red disconnects for all utilization equipment?

Ask inspector for a code reference? Or is it a city official requesting it?
