Is a generator automatic transfer switch, service rated, 200 Amp, installed after the meter and before the existing service disconnect, considered the service disconnect?

If it is service rated it will have the service disconnect built in.

Yes, if it is SUSE rated as you indicate. You need to move the GES to the ATS and run a EGC to the old disconnect and remove the main bonding jumper though. Have you considered just removing the old service disconnect?
