I haven't/can't find to date any specific requirement that would include a disposal receptacle. The distance to the sink intent I believe would be as measured from the sink edge(of Opening) and not the external bottom tho true enough it doesn't say that.This is another judgement call in my opinion.

That is correct that it has no exceptions. This section should read excluding under or over cabinets receptacles behind doors when there is a counter outlet within 6' of the sink.
 
210.8(B)(5) Sinks ? where receptacles are installed within 1.8 m (6 ft) of the outside edge of the sink.
Commentary: As is the case for dwelling units, GFCI protection is now required for all 125-volt, 15- and 20-ampere receptacles installed within 6 ft of the outside edge of all sinks that are located in other occupancies. This requirement covers receptacles installed near sinks in lunchrooms, janitors' closets, classrooms, and all other areas that are not covered by the bathroom and kitchen provisions of 210.8(B)(1) and (B)(2).
