MEG Test(Insulation) Question

When making a form for the test, what kind of info should I include? I do not have a proper form and need to make one.

I have #8, #10, and #12 wire that will be running at 480V on motors. Wire was just pulled.
With a Megaohmmeter, I put 550VDC on them.
Results for all were the same, 550megaohms at 526VDC.
 
Usually the forms I deal with have location of equipment , size , insulation type and length of wire . Ambient temperature ( makes a big difference in readings ) Make , model and cal date of megger .test voltage ( 1k for 600v cable ) . A reading from every conductor to ground and to each other .
We meg before we pull it off the reel and again after we pull it .
 
I think it would be useful to include pass/fail values. That may change if it is a cable, a motor or a switchgear.
I agree. FWIW, I don't believe I have ever seen a test record that includes spaces for all the pertinent data that should be on such a form. The first link is to one used by a former employer of mine, with logo and header removed. I just posted it to spur ideas, such as yours.

Ambient tempature is a huge factor, motors should be corrected to 40C, everything else to 20C.

Besides that and everything else mentioned the biggest thing would be time of test and values at those times. Motors should have a reading recorded at 30 seconds, 1 minute, and 1o minutes with DAR and PI ratios calculated.
