Grounding conductor in conduit.

I have a question about the code NEC. Are all conduit runs with ungrounded conductors supposed to have equipment ground or not? I have been installing a separate ground or equipment ground for a while when ever I run conduit I work in a healthcare facility, however I have found some older installations where there is no
separate equipment ground. If it is a code requirement where is it?
 
Read up on 250.118 for Types of Equipment Grounding Conductors. If your conduit system qualifies as an equipment grounding conductor and the specifications don't require it, then its up to you or your boss's policy. For health care facilities, see 517.13(A) and (B)

One of the strange twists with health care facilities I never could understand is the redundant ground for branch circuits but not for feeders.
I wonder if the #10 that is usually bonding the different panels together is the reasoning??
