Custom Arc Flash Label

I am developing an arc flash label under etap arc flash program, my question:
am I allowed to use company name, approving engineer and other information
on the label? (see attachment).

I don't think it is prohibited. Whether it is advisable is something else.

You are allowed
It will help them remember who to call if they need an update, however, it will also remind them who to call if there is an accident.

We explicitly leave our identifying information off safety labels. Once you walk away from the install you relinquish control of the system. I don't want our name front and center when something happens 10 years from now after 3 other electricians have come and gone. We have separate labels for advertising.
