Disconnect for a Small Pump

Is a disconnect required for a Radiant Floor Circulating Pump - 120/1/60 - 0.125 HP?

The closest I can find is NEC 430.109 (B) saying for stationary motors of 1/8 HP or less do not need a disconnect and that the branch circuit breaker can act as the disconnect. Is there a similar code reference for small pumps?

Every motor requires a disconnect within sight of the controller and motor. For a small motor, the disconnect can be a cord and plug.
The branch circuit breaker would be allowed if it was capable of being locked out.

The branch circuit breaker would be allowed if it was capable of being locked out.

does this apply to ceiling fans (~1.2A) that use remote control for the sole on/off (connected to gfi breaker) ??

Every pump contains a motor.
In some specific cases, such as HVAC equipment, the NEC treats an assembly containing a motor differently than a separately installable motor, but this is, IMHO, not one of those cases.
