When you encounter a doorway in a living room, does the 12? spacing between receptacles reset itself back to 0? so that the doorway opening now requires the 6'?

That is correct-- 6' max from an opening

Yes, you would need a receptacle within 6', as measured along the wall, from the opening.

Yes, a closet door opening would count as well.

Basically doors, appliances, fixed cabinets, sliding door type breaks on the wall will restart the 6' rule again. However, some interpretation will apply.

IMO, it would depend on the arrangement. If the area on the back side is a dinning room or breakfast area then the outlets on the counter may work as long as they are not 12' apart. However if the area backs up to a living room then IMO you need one there that is not part of the small appliance branch circuit.
