Repair Requirement for Broken Bolt-Loc Switch

I have a customer who has a Square D Bolt-Loc switch with a broken operator handle. It appears the switch is inoperable (though in-service feeding an important part of the plant).
We all know it should be repaired.
I am just looking for a reference in NEC or 70B that says it MUST be repaired.
I can't find anything.
Anyone got chapter and verse for me?

Have you looked into NFPA70E, it regularly mentions 'properly maintained' equipment.
How about OSHA and one of its LOTO requirements?
 
NEC probably has nothing to do with this.

What are the consequences If you don't repair the switch and it fails?
 
It is a fused switch, so the fuses will determine the arc flash category. I suppose the consequences of not repairing the switch are that it may not function properly for energy isolation, or once opened it may be impossible to close, resulting in an extended unplanned outage.
