It doesn't appear to have a hold down or a way of turning it off.....

OK, I see that now. That's a new one one me.

Also, does the top 20 amp breaker look tripped?

These fail pretty often in my opinion.

I have never seen one before.
So when it fails it just doesn't work?
If I had more time I should have checked out what fed it.
I don't know that I've seen a panel that big,,200 that didn't have main lugs atleast.

Are these sub feed lugs that much cheaper than a 3 phase breaker? I never use them.

OK, It's a sub feed lug. What was the advantage/reasoning to do it this way vs a separate lug for each phase like is common in MLO panels today ? Or let me ask the opposite question. Why was this sub feed lug method discontinued in favor of lugs in a MLO ?

I'd think back feeding a 200 amp panel just became a bad idea.
I know zero about how panels are designed but it seems like where the main lugs or main is now looks like thicker metal. Which could make sense.
A 100 amp panel is still bus fed. .
