EMT EGC Question

When pulling a separate insulated EGC in lieu of using the EMT for an EGC, is it a code requirement to bond the EGC to each junction box? The wire EGC would then only serve the loads. I'm having trouble finding the correct article in the NEC.

If the wires aren't spliced in the box then the EGC doesn't need to connect to the box.
When there are splices, then the EGC must be terminated to the box.

I have heard some interpretation that if you leave a service loop of the ungrounded conductors in a box, then the ground has to be bonded as well. FYI.
