It would be just around the conduit or if you were using fire alarm MC cable. if in doubt check with the AHJ. or the project engineer.

Most penetration details I have seen show both.

You left out key components.

What is the rating of the assembly you are penetrating?

Type of material? Etc.

You should have an assembly detail with you when you do any type of firestopping. This is often an after the fact item and rushed and done incorrectly, but it is of great importance.

If you look at any of the major firestop manufactures 3m, Hilti, you can download an assembly and it's easy just to follow the detail.

Also when the inspector asks for the detail you can show him/her to avoid any confusion in the field.

Thru a fire rated wall is it code to fire stop outside and inside the conduit? thanks

Assuming your wiring is in emt all you need to do is fill an annular space around the emt. The sizing of the annular space is a function of pipe diameter & material.

If you're talking about putting something through a sleeve, then you would need to pack the sleeve and well as any space around the sleeve.

Article 770 refers back to 300.21. If you are running a cable through an EMT sleeve, yes, you caulk inside (between wire and EMT) and outside (between sleeve and wall/floor). As mentioned, 3M, Hilti, and others will have diagrams of just about any kind of penetration you might need to make. STI also has many specialty products.
