Grounding/Bonding at a Rectifier

I'm wondering about code violations for a remote site rectifier. I have a 240v 3-wire drop to a disc where the ungrounded conductors are fused. Then 2 black ungrounded conductors and 1 white grounded conductor go to rectifier enclosure where the ungrounded black wire land on a 2-pole bkr. The rectifier enclosure is where the ground rod and GEC are at as well as a 120v ckt for a recp. Can anyone please tell me if this is a code violation? I thought the main bonding jumper and the EGC and GEC and ground rod where to be at the first available disconnecting means and then kept seperate from there on out. Am I missing something??
